Informed Consent
If you’re onboarding yourself through the mobile app
During onboarding in the mobile app, you'll see a check box that says "I have read and agree to the Clinical Services and Practice Policies Agreement".
To review and/or print the terms in full before agreeing, tap the "Clinical Services and Practice Policies Agreement" URL. This will open up your browser to reveal the full terms.
When finished reviewing, close the browser to return to the onboarding page. Check the box to agree to our Informed Consent terms.
If you were assisted with onboarding over the phone by one of our Health Guides or Support Agents
After downloading and opening the mobile app for the first time, you will see a pop-up requesting that you accept our Terms of Use and Clinical Services and Practice Policies.
Tap the URLs to review the terms in full. Once you complete your review, tap Accept to agree to our terms.
Informed Consent for Therapy and Care Navigation
For members enrolling in our Therapy Program, you will see an additional item on your To-Do List on the Home screen to complete your Informed Consent for Therapy. Tap to open the form.
Review the terms by tapping the URLs for Privacy Policy and Terms of Use. To agree to the terms, type your name in the box below then tap complete.
Form: Member Contact Information
Your “Form: Member Contact Information” will be listed on the "To-Do List" on the Home screen.
- Open the Home screen of the Vida app and tap “See All” under the “To-Do List”
- Tap “Form: Member Contact Information”
- Tap each empty box in the form. Type in the details with your on-screen keyboard (this will pop-up when you tap the empty box)
- After typing in all the information, dismiss your keyboard by tapping “Done” (or the down arrow on Android), then tap “Complete”
- Note that responses cannot be changed after sending. When ready, tap “Send” on the pop-up to send the form.
- Your to-do list form will now show a green check-mark indicating the form was completed.
