Your Vida Medical Provider may ask you for medical records or documents. Follow the instructions below to share your documents.
First, make sure your medical documents include all the information requested by your provider. This typically includes:
- Your name
- Your date of birth
- Date of test or prescription
- Test or prescription name
- Test result, value, or dosage
- Name of the prescriber
- Your BMI at the time
Your provider will ask that you share this with them. To share with the provider:
-
Send a PDF or take a picture
- Paper results: Lay your paper flat on a surface and ensure you have lots of overhead light. Open your phone camera, hold it over the page, and move your phone until the results fit on the screen. Take a few pictures in a row in case one is blurry.
- Online: Open the lab results in the medical office or lab online portal. Many have an option to “Print” your results. You can use the print function to save an electronic PDF to your phone or computer, or you can take a screenshot. It’s okay if all the results don’t fit in one picture. Take as many pictures as needed to show all the results. Make sure one of the images has your name, date of birth, and the date of the lab visit or prescription.
