The Whole Health (WH) program is a great choice for those looking to manage health conditions from head to toe. This program can help you to manage your diabetes, high blood pressure, high cholesterol, and more.
Your coach will help you make a plan to help you navigate your health journey – one step at a time.
Program Details
What can you expect in this program?
- Weekly video/call sessions with your Registered Dietitian or Certified Diabetes Educator and ongoing Chat support
- A personalized wellness plan made just for you
- Daily tips, tools, and easy action steps (including videos and demonstrations)
You may also be eligible to receive a device in this program, like a smart blood sugar meter, weight scale, or blood pressure monitor.
Who’s eligible for the program?
This program is available to qualifying members with chronic conditions including diabetes, high blood pressure, and high cholesterol. You do not have to have diabetes to participate in this program, however, your provider will be the best person to help you determine if the program is right for you.
This program is not available for those under 18 or currently pregnant. Your coach will also ask about other conditions and medical history to ensure the program is appropriate for you.
Type of provider(s). You will work with one or both of the below providers:
Registered Dietitian (RD)
Vida’s registered dietitians (RDs) will use their nutrition and diabetes management expertise to help you apply and maintain healthy lifestyle choices that support your goals for the program.
Certified Diabetes Care and Education Specialist (CDCES)
Vida’s CDCESs are specially trained to understand and help manage high-risk diabetes members. Depending on a member’s needs, a CDCES guest coach may step in to assess your diabetes self-management knowledge and potential risk factors or to provide specialized support and education while your dietician is away.
Program Duration
1-year program broken down into phases:
- Onboarding (week 1): A 45-minute initial consult with your RD to discuss your goals while participating in the program.
- Active (weeks 2-10): Weekly to bi-weekly sessions with your RD
- Maintenance (weeks 11+): Monthly video/voice calls with your RD for continued support.
After 6 sessions with your coach, we’ll look at your progress. Then we’ll adjust your program to be the best fit for you and your goals.
Program Focus
Throughout this program you will hone in on your health behaviors. Things like eating healthy, taking medicine, being active, mindful awareness, understanding your key health metrics and trends, and sleeping well to name a few.
You and your coach will work to understand and improve your current health goals and incorporate new ones.
Program Goals
Program goals are personalized based on your individual condition(s), needs and health goals. For example, the primary focus for members with moderate to severe diabetes is an A1C reduction to 7% or lower. Members with high blood pressure or cholesterol may focus on lowering blood pressure, or losing weight.
